Appointments

What should I bring to my first appointment?

We want to make your first visit with us a pleasant one.  We ask that you bring your insurance card(s), a photo ID; your registration form(s) that were either mailed to you or printed from this website (Click here for forms) 

Upon each visit with your physician we collect the appropriate co-pay amount and deductibles.  Many of our locations accept Visa, MasterCard, American Express, cash and personal checks for your convenience.

Can I get an evening appointment?

Many MPM Primary Care locations have evening hours.  Please review the office locations on this website for each location’s office hours.

What is the 4DOC Line?

The 4DOC phone line is another convenient way for you to find a MPM Primary Care doctor in your area who may be taking new patients or accepting your insurance.  Please call (727) 535-4DOC or (727) 535-4362

What happens if I miss an appointment?

Because we are concerned about you and don’t want you to forget, Morton Plant Mease Primary Care, Inc. now follows the process below if you miss your appointments.

·         1st missed appointment: we will call you and offer to reschedue the appointment.  You may be charged a missed appointment fee of up to $35.00.  

·         2nd missed appointment:  we will call you and offer to reschedule the appointment.  You will be sent the group's 2nd missed appointment reminder letter.  You may be charged a missed appointment fee of up to $35.00.

·         3rd missed appointment:  this could result in your discharge from our practice and a missed appointment fee of up to $35.00.  You may be asked to find another physician outside of the Morton Plant Mease Primary Care, Inc. group of physicians.

So – DON’T BE A ‘NO-SHOW’ – let’s work together to provide you with the care you deserve.

Billing

The billing question site pertains to the Primary Care Physician practices only.

Who do I contact regarding my bill?

Our central billing office (CBO) handles all of our patient's billing needs, from the electronic filing of insurance claims and patient statements to assisting our patient's with questions regarding their bill.

The CBO can be contacted at (727) 532-0002 or 1-877-532-0002, where one of our Customer Service Representatives will be happy to assist with any billing questions.

What insurance plans does my doctor accept?

MPMPC Primary Care Physicians participate with Medicare and most Commercial insurance plans.  To find out if your physician participates with an insurance plan please call the 4DOC phone line at (727) 535-4DOC or (727) 535-4362.

Why does it take so long to receive a bill from my doctor?

The Central Billing Office tries to make sure no patient is billed until all insurances pay their obligations. While we do not like to wait to bill a patient, we also do not want to bill a patient in error.

Which hospitals do MPMPC Primary Care physicians admit their patients to?

Should the need arise for you or a family member to be hospitalized; the MPMPC Primary Care Physicians have privileges at the following hospitals:

Morton Plant Hospital, Clearwater, FL

            Mease Dunedin Hospital, Dunedin, FL

            Mease Countryside Hospital, Safety Harbor, FL

            Morton Plant North Bay, New Port Richey, FL

 

Please check your provider’s information page for the hospitals he/she is affiliated with.

My physician does not go to the hospital, who will take care of me if I have to go to the hospital?

Your MPMPC Primary Care physician may arrange for your hospital care to be managed by a team of dedicated "Hospitalist” physicians. These “Hospitalists” spend 24 hours each day in the active management of MPMPC patients in the hospital. Should you be hospitalized for any reason, these hospitalist physicians will manage your care while you remain an in-patient.

The Hospitalist is able to insure that you receive both timely and expert treatment throughout all phases of your hospital stay, keeping your MPMPC primary care physician constantly updated on your progress.

How do I get a copy of my records from MPMPC ? 

To receive a copy of your medical records please download the authorization form, complete the information requested and submit it to your MPMPC provider for processing.
